AI-based decision support systems in community home care for older adults with multimorbidity: a scoping review

Older adults living with multimorbidity often receive complex and fragmented care in municipal home care settings. Healthcare professionals in these settings are required to make frequent and complex decisions related to assessment, prioritisation, and coordination of care across conditions and services. At the same time, demographic changes and workforce shortages increase pressure on municipal health services to deliver high-quality and efficient care in patients’ own homes.

Artificial intelligence (AI) and digital decision support systems have been introduced as potential tools to support clinical and organisational decision-making in home care. However, existing reviews primarily focus on hospital settings, single-disease populations, or broader digital health interventions. There is currently no comprehensive overview of how AI-based decision support systems are conceptualised, implemented, and evaluated in municipal and community home care for older adults with multimorbidity.

A scoping review is therefore appropriate to systematically map the existing literature, identify knowledge gaps, and inform future research, implementation, and policy development.
