Den præoperative helbredstilstand hos patienter med primær hoftearthrose

SummaryThe health condition of patients with osteoarthrosis was examined before Total Hip Arthroplasty. This was done with a condition-specific outcome measure (Harris Hip Score), two generic outcome measures (Short Form 36 and EuroQol - 5D) and Quality Adjusted Life Years (QALY). 17 patients with a mean age of 62.8, from 37 to 82 years, were included at the Department of Orthopaedic, University Hospital of the Aarhus University over a five-month period. This is statistical significant (P < 0.05) younger than the data from the Danish THA Register (68.1 year). 59 percent of the patients were women. The Harris Hip Score was 50.0, which is statistical higher than the data from the Danish THA Register (43.0). The scores on three out of the four SF-36 physical dimensions were significant lower than agematching data, and comparable with data for other patients with chronic diseases. Similar scores were calculated for EQ-5D, this is comparable with RA-patients (function class II) and patients with osteoarthrosis in the knee. The QALY were calculated to 0.64 or 0.63 depending on whether there were used a fourteen years period in the Time Trade-off method, or a patient specific statistical lifetime. The correlation between these methods was 0.873. Thus indicates that in this project the "cardinal utilities under certainty" measured by the TTO-method, has a linear relationship to "vNeumann-Morgenstern utility". The QALY measured in this project is high, compared to data from the literature. The correlation between Harris Hip Score, SF-36 the four physical dimension and EQ-5Dutility is pretty good. On the other hand the correlation between EQ-5Dutility and EQ-5DVAS is surprisingly poor.It is concluded, that condition-specific outcome measure and generic outcome measures in combination gives a better picture of the patients and their situation.
