E-cycling as Intervention in Forensic Psychiatry: A Constructivist Grounded Theory Study of the Importance of Interaction for Recovery

    Abstract

    Patients in secure forensic psychiatric care have increased comorbidity and shorter life expectancy. Physical training tailored and individualized to accommodate individual forensic psychiatric patient needs is crucial for successful rehabilitation. Physical training aims to increase physiological and
    mental health, though difficulties in motivating patients for physical training highlight the complex nature of caring for individuals in this context. Therefore, it is essential to understand and support patients' motivation to engage in rehabilitation interventions. However, few studies have currently
    demonstrated how this is to be done in practice.
    Objectives: To study patient-professional interactions during e-cycling and their relationship and impact on patients’ motivation in Danish medium secure forensic psychiatric hospital wards.
    Methods: The design was constructivist grounded theory. Data was collected during 8 participant observations of the intervention and one focus group interview with 6 professionals. The study included 5 patients and 6 professionals.
    Results: Four categories of patient professional interactions were
    identified shown in Table 1.:
    1) Distributive interaction
    2) Outcome-focused interaction
    3) Relationship-focused interaction
    4) Accommodating interaction
    The categories emerged to a theory Together for a Healthier Life. Each category based on micromoments was driven by a common specific form of; goal, motivation, focus, and communication. Depending on the interaction.
    Conclusion: It is essential to acknowledge the productive potential of focused interactions in micro moments to accommodate the motivational needs of forensic psychiatric patients. Needs and motivations are never stationary but may fluctuate. Therefore, professionals must be continuously aware of the needs expressed in micro-moments and reflexively adapt to accommodate any changes in motivation as they unfold. Adopting an increased focus on micro-moments may inform future research and provide. knowledge to heighten motivation among forensic psychiatric patients.
