Mapping of primary care interventions and evidence of interventions targeting community-dwelling older adults with chronic multimorbidity: a scoping review

Objective: The objective is to identify gaps in the knowledge base related to interventions targeting older community-dwelling people with chronic multimorbidity in primary care settings.
Introduction: The rising prevalence of multimorbidity poses significant challenges for both individuals and health systems. Multimorbidity affects more than one-third of the global population, leading to reduced quality of life, increased mortality, higher healthcare use, and thereby significant healthcare expenditures. Traditional disease-specific models of care are failing to address the complex needs of these individuals, resulting in fragmented services and treatment burden.
Methods: The review followed the JBI methodology for scoping reviews. A comprehensive search was conducted in PubMed, Embase, and CINAHL, to identify peer-reviewed articles. Screening of titles and abstracts, as well as later full texts, was conducted by at least two independent reviewers. Data were extracted using a customized tool developed by the authors and narratively summarized and presented in tables. The Consolidated Framework for Implementation Research (CFIR) was used to analyze reported facilitators and barriers to implementation.
