Abstract

250 nurses with documentation expertise within the fields of management, clinical practice, research, development and education defined their criteria for requisite documentation based on five different perspectives: the patient perspective, the technology perspective, the organisational perspective, the nursing perspective and the educational perspective. The resulting data underwent systematic text condensation, which identified eight themes:
1) The patient perspective is the joint responsibility of the patient and nurse
2) Documentation must be produced jointly with the patient
3) Documentation going forward will require changes to routines and habits
4) Managers must assume joint responsibility for instilling a dynamic culture in which documentation is important
5) The requisite documentation must be prioritised and clarified
6) Competence building and training must be practice-based
7) The creation of a National Strategy for Standardised Intersectoral Documentation
8) Documentation should be a consistent topic in nurse education modules. Based on the analysis, the conclusion is that patient involvement, management involvement and a nationwide strategy constitute the three cornerstones for underpinning the requisite documentation practice.
